Patient Reminders

For Existing Patients:

Existing Great Lakes Dermatology patients should bring:

  • Your photo I.D.
  • Your current insurance card
  • Payment for your co-pay, if your insurance plan requires one
  • Payment for any outstanding balances you have

For Patients Who Require a Prescription Refill:

We ask that all patients contact their pharmacy to have them fax a prescription refill request to our office. Please allow 1-2 weeks prior to running out of your medication for a prescription refill. Any pre-authorizations that may be required by your insurance company could delay the refill without timely notice.
