Patient Resources

Below, please find useful information for your upcoming appointment—whether you've just scheduled your first office visit, or are a current medical or cosmetic dermatology patient.

For New Patients

If you are a new Great Lakes Dermatology patient and this is your first appointment with us, you should bring:

  • The new patient packet you received in the mail (or download the online forms)
  • Your current insurance card
  • A photo ID
  • A list of your medications
  • Payment for your co-pay, if your insurance plan requires one

If you have not received your new patient packet in the mail, please arrive at least 15 minutes early for your appointment so that you can complete the necessary paperwork.

For Existing Patients

Existing Great Lakes Dermatology patients should bring:

  • Your current insurance card
  • Payment for your co-pay, if your insurance plan requires one
  • Any prescription refills needed at the time of your visit

 

For Patients Who Require a Prescription Refill

Please allow 1-2 weeks prior to running out of your medication for a prescription refill. Any pre-authorizations that may be required by your insurance company could delay the refill without timely notice. Also, please have the name of your medication and the name and phone number of your pharmacy available when contacting us regarding your refill.
